Use this form to apply to be a SunSmart early childhood centre OR renew your SunSmart status.
Before filling in this form
- You must have a written skin protection policy for your centre. You will not be able to submit this form without your policy attached.
- Make sure it includes the main points recommended by the Cancer Council - use the sample policy and checklist as a guide.
- Make sure you have all the necessary details available. It should take about 10 minutes to complete.
What happens next?
- You will receive a confirmation email immediately after submitting your application.
- New applications
- New applications will take approximately 4 weeks to process.
- If your centre meets the SunSmart Centre criteria, you will receive a Congratulations kit and SunSmart Centre agreement to sign.
- You will be sent a metal SunSmart Centre sign (90 cms x 60 cms).
- Renewals
- If you centre meets the SunSmart Centre criteria, you will receive a Congratulations letter and a SunSmart Centre agreement to sign.
Becoming a SunSmart centre tells the community
that you are committed to the future health of its children.

Member details will only be used for the purposes of Cancer Council SA’s SunSmart Schools program. All school information will be kept private and confidential, and no identifying information will be used for statistical purposes.
