Use this form to apply to be a SunSmart Family Day Care OR renew your SunSmart status.
Before filling in this form
- You must have a written skin protection policy for your Family Day Care. You will not be able to submit this form without your policy attached.
- Make sure it includes the main points recommended by the Cancer Council - use the checklist as a guide.
- Make sure you have all the necessary details available. It should take about 10 minutes to complete.
What happens next?
- You will receive a confirmation email immediately after submitting your application.
- New applications
- New applications will take approximately 4 weeks to process.
- If your Family Day Care meets the SunSmart Family Day Care criteria, you will receive a Congratulations kit and SunSmart Family Day Care agreement to sign.
- You will be sent a SunSmart Family Day Care certificate
- Renewals
- If you school meets the SunSmart Family Day Care agreement, you will receive a Congratulations letter and SunSmart Family Day Care agreement to sign.
Becoming a SunSmart Family Day Care tells the community
that you are committed to the future health of its children.

Member details will only be used for the purposes of Cancer Council SA’s SunSmart Family Day Care program. All Family Day Care information will be kept private and confidential, and no identifying information will be used for statistical purposes. If you are concerned, contact Cancer Council’s SunSmart Program on 8291 4111.
