Referring a patient to Cancer Council SA Accommodation Service

To ensure every guest receives the best possible care and support they need during their stay, please complete the below referral form and confirm the booking before sending a guest to Cancer Council SA.

Key Requirements:

  • Notice Period: Where possible a minimum of 48 hours' notice for bookings to ensure we can adequately prepare for each guest's arrival.
  • Referral Form: A referral form with targeted questions must be completed. While we still accept short notice referrals, key questions must be answered before booking confirmation.
  • Guest Requirements: Guests must be able to care for their physical needs independently. We provide emotional support, but guests need to manage their own shopping, cooking, and travel. Evening meals are available at a subsidised rate on weekdays, and limited transport to treatment is offered.
  • Carer Requirement: If guests are unable to care for themselves, they must be accompanied by a carer. If care needs are too high, our services may not be suitable.
  • Assessment: Our Social Workers will review mobility, daily living needs, and independence levels before confirming bookings. Any concerns will be explored further to ensure the best outcome for our guests.
  • Hospitalisation Review: Guests returning from hospital with changed care needs or medical conditions should be assessed by our Social Workers to determine if it is appropriate for them to return.

Completed referral forms can be forwarded to stay@cancersa.org.au
For further information please do not hesitate to speak with our social workers.
